1. Recommended: Interactive PDF Format
   
   Instructions: If you click the link above, your computer will either open it automatically within your browser window, or ask you to save it to your computer. Either way, you need Adobe Reader/Acrobat to view it.
   Once the file is open, begin filling out all the necessary blanks.
   
   There are two ways to submit it to Maple Leaf Funding.
   1) Once you are done filling it out, save the Application to your computer's hard drive (ie. My Documents). Then open your Email application (ie. Microsoft Outlook or similar software). Send an email to merle@mapleleaffunding.com and be sure to attach the PDF Application that you just saved to your hard drive (ie. My Documents)
   
   2) Once you are done filling it out, simply go up to the top left corner of your browser window, and click on FILE >>> PRINT. (There might be a printer icon within the Adobe Reader software as well.) Then fax it to 254-947-1941.
